Organ, eye, and tissue donation is about LIFE. Those who have chosen donation are true heroes. They have selflessly passed along the gift of life to another individual. The Celebration of Life Monument was developed and built by Utah’s Donate Life Coalition (which includes Intermountain Donor Services, Utah Lions Eye Bank, and the University of Utah Whole Body Program) as a memorial to donors and their families, as well as a motivator to others to save and enhance lives through donation. The first names were etched on the monument in 2004 and 500-600 names are added each year. If your loved one was an organ, eye, tissue, or whole body donor upon death, his or her name is eligible for engraving on the Celebration of Life Monument. If you are a living donor— kidney or liver, your name is also eligible for engraving on the monument. Please note that names of donors are not automatically included on the monument. If you would like to have your loved one included on the Celebration of Life Monument, please fill out the name submission form here or contact Donor Family Services at 1-801-478-7350 or firstname.lastname@example.org. The monument is located on the SE corner of Library Square, 300 E 500 S, SLC, UT. Any donor who has a connection to the Intermountain area may have their name engraved on the Celebration of Life Monument free of charge.
